Search "intermittent fasting for women" and you will find two confident camps: one insisting fasting wrecks female hormones, the other insisting sex makes no real difference at all. The honest answer sits in between, and it starts with an uncomfortable fact about the research itself. For much of intermittent fasting's rise in popularity, human trials have skewed toward male participants, and even studies that enrolled women often did not analyze results by sex. That gap does not prove fasting is unsafe for women. It means many of the strong claims circulating online, in either direction, are more certain than the underlying evidence actually supports. This article walks through what verified research does and does not show about intermittent fasting, menstrual cycles, and reproductive hormones, and how some clinicians and researchers suggest women approach getting started.
What We Actually Know About Sex Differences in Fasting Research
A 2022 systematic review in Frontiers in Nutrition gives one of the more concrete looks at this gap. The authors screened 381 articles on Ramadan intermittent fasting, a population-scale fasting model practiced by both sexes, and found that only about 10 percent of those screened articles reported results broken down by sex at all. Within diabetes-focused research specifically, that figure dropped to roughly 4 percent, even though the underlying studies included both men and women. Of the 38 studies that made it into the final analysis, covering more than 25,000 patients, only 44.4 percent of participants were female. The review's core conclusion was not that fasting is dangerous for women, but that researchers have rarely asked the sex question in the first place.
A broader 2026 narrative review in Nutrients makes a similar point from a different angle. The authors note that direct sex-comparative fasting studies remain scarce, and many public conclusions about "women and fasting" are stitched together from separate male-only and female-only studies rather than head-to-head trials using the same protocol. Interestingly, they also point out that the imbalance runs in more than one direction: mechanistic fasting physiology research has often excluded women, citing menstrual cycle variability as a confounding factor, while many real-world intermittent fasting weight-loss trials have skewed female and underrepresented men. Neither literature was built to answer "does this work the same way for both sexes," so neither one fully can.
What this means in practice is that blanket statements deserve skepticism. "Fasting is unsafe for women" overstates a body of evidence that includes studies showing no disruption at moderate intensity. "Fasting affects everyone identically regardless of sex" overstates a body of evidence that increasingly points to real physiological differences in how men and women handle extended energy restriction. The more accurate, less satisfying summary is that responses appear to depend heavily on fasting intensity, duration, body composition, and individual hormonal status, and that women have been studied less often and less specifically than the confidence of most online advice would suggest.
Fasting and the Menstrual Cycle: What the Evidence Shows
The most direct experimental data on this question is also some of the oldest. A study published in Clinical Endocrinology tracked normal-cycling women through a 72-hour fast during the follicular phase and found no discernible disruption to luteinizing hormone (LH) pulse patterns, ovarian follicle development, or overall cycle length, despite clear metabolic stress markers elsewhere in the body. The researchers concluded that the female reproductive system, at least under that specific short-term, moderate protocol, showed more resistance to acute caloric restriction than has been observed in some studies of men. It is an important data point, but it is one study of one protocol, and it should not be read as a blanket guarantee that longer or repeated fasts behave the same way.
A 2022 review in Nutrients that pooled the available human trials on fasting and reproductive hormones adds nuance rather than resolution. Across the studies reviewed, intermittent fasting was associated with lower androgen markers, including testosterone and the free androgen index, along with higher sex hormone-binding globulin (SHBG) in premenopausal women with obesity, and this effect appeared more consistently when eating was confined to earlier in the day. Estradiol, LH, and FSH, by contrast, were largely unchanged across the small number of studies that measured them, though the review's authors were candid that very few trials existed, sample sizes were small, and durations were short. In men, the same review found that fasting could lower testosterone in lean, physically active young males, without measurably impairing muscle mass or strength gains, which is a useful reminder that hormone shifts on a lab panel do not always translate into a functional problem.
Where the two sexes clearly diverge is glucose and insulin handling. The same Ramadan-fasting systematic review that exposed the sex-reporting gap also found a sex-specific metabolic pattern in the studies that did report by sex: blood glucose decreased and hypoglycemia increased in women only, while insulin resistance improved in men only. The 2026 Nutrients narrative review offers a possible physiological explanation, describing women as generally showing more flexible lipid metabolism and better preserved insulin sensitivity despite fasting-related increases in free fatty acids, whereas men tend to rely more on carbohydrate metabolism and show a tighter link between fat mobilization and insulin resistance during fasting states. The same 2026 review also describes women as having greater neuroendocrine sensitivity to energy deficit than men, meaning fasting can, in some circumstances, suppress LH pulsatility and disrupt normal cycling, particularly in women who are already lean or under added physical or psychological stress. At the same time, the review notes that short-term fasting in women who are otherwise energy-replete can shift reproductive hormone dynamics without necessarily impairing ovulation. Put simply, the hypothalamic-pituitary-ovarian axis appears to be sensitive to how much of an energy deficit fasting creates on top of everything else going on in a woman's life, not to fasting as a fixed, one-size-fits-all exposure.
Practical, More Conservative Starting Points for Women
There is no major medical body that has published a formal, sex-specific intermittent fasting protocol for women, and it would be dishonest to imply otherwise. What exists instead is a loose but recurring pattern in the research and clinical guidance that does address women directly: start smaller, ramp up slower, and pay attention to how the menstrual cycle interacts with the fasting window.
The clearest example comes from breastfeeding guidance. Cleveland Clinic has published clinical advice stating that a registered dietitian generally does not recommend intermittent fasting for people who are breastfeeding, since nursing typically requires an additional 350 to 500 calories per day and a shortfall can affect both milk supply and energy levels. For those who choose to fast anyway, later in breastfeeding once a baby is eating more solid food, the same guidance points toward shorter fasting windows, often in the 12 to 14 hour range, done in coordination with a health professional rather than a standard 16:8 schedule adopted without adjustment.
Outside of breastfeeding specifically, the more general, honest takeaway from the reproductive hormone research above is that women who are lean, highly active, or already under other physical or psychological stress appear to be the group where more aggressive fasting is most likely to interact with hormone signaling. For that group, a gradual approach, such as starting with a 12-hour eating window before considering anything longer, and tracking how energy, mood, and cycle timing respond over a few weeks, is a more conservative and better-supported starting point than jumping straight into extended fasts or alternate-day protocols. Given how much of the reproductive hormone research points toward energy deficit and leanness as the variables that matter most, a few warning signs are worth taking seriously rather than pushing through. A missed period, a menstrual cycle that becomes noticeably shorter, longer, or irregular after starting a fasting routine, or new symptoms like persistent fatigue, hair thinning, or mood changes that track with your fasting schedule are all reasons to loosen the protocol and talk to a clinician, not reasons to fast harder to "push through" a plateau.
Women with a personal history of disordered eating deserve particular caution. Structured food-timing rules can be clinically useful for some people and a relapse trigger for others, and intermittent fasting's rules-based structure can resemble patterns that are risky for someone recovering from restrictive eating. If that history applies to you, this is a conversation to have with a healthcare provider or eating disorder specialist before starting, not after a problem shows up.
The same caution applies to anyone trying to conceive, pregnant, or breastfeeding.
